How to order an Eyecare Plan?

Salveo ltd ensure that all requests are dealt with efficiently and in a timely manner therefore all requests must be submitted using the online form provided.

What information do we need?

Once the online form is completed, the requests are then fed into a database which is used to keep track of the eyecare plans issued by the employer. The email address provided is used for sending reminders to ensure employees use their plan before they expire.

To prevent abuse of the eyecare plan, the information provided is used to personalise the eyecare plan for the individual using it, this way no unauthorised employees can use the entitlement.
