2. Directory of UK Affiliated Opticians. The Directory lists all Affiliated Opticians (over 1,700).It also indicates with symbols which of these opticians hold sample frames and / or give discounts on any private purchases. Optician on-line directory with easy search facility by Town, County, Post Code or Optician Name: Search Directory
3. Service Appraisal Card This pre-paid card enables feedback to be carefully monitored.

HOW IT WORKS

• Employer issues a Sight Test Plan.
  Section 5 of the Health & Safety Display Screen Equipment Regulations 1992 gives the employers a duty to ensure the provision of appropriate eye and eyesight tests on request:
1. to their employees who are already users; and
2. to people who are being recruited to be user (including existing employees being transferred to work that will make them a user); also
3. for users at regular intervals after the first test, provided that they want the tests
• Employee makes appointment with an optician from those listed in the accompanying directory (over 1,700 throughout the UK). There is an on-line directory with easy search facility by Town, County, Post Code or Optician Name: Search Directory
• The optician will carry out a full ophthalmic eye and eyesight test.
• From the sight test results, the optician will complete the Recommendation Certificate detailing the exact purpose of any spectacles required, including the type of lens (single vision, bifocal or varifocal), and the exact purpose of the lens (i.e distance vision, intermediate VDU viewing, reading etc).
• To assist employers in understanding the Optician’s recommendation, a separate Explanation Sheet is provided with each Sight Test Pack.
• The employee is instructed to return the Recommendation Certificate to their Employer.
• Upon reading the Recommendation Certificate the Employer may decide to issue a Spectacle Pack to allow for Company funded spectacles to be provided.
